Payment Options
There are multiple payment options to help with the cost of home care. We will assist you in identifying any programs for which you qualify. We will also take care of the necessary paperwork for reimbursement. Below are examples of payment options:

  • Medicare
  • Medicaid
  • Private Insurance
  • Worker’s Compensation
  • Private Payment

Our Process
Our patients are received by referral and have orders for home care from their primary care physician. Transitions Home Health and Private Care will use the insurance information provided by the patient to obtain payment for services received. Payments for services should not be given to any of our staff that visits clients.

Did You Know…
You might not be aware that insurance, including Medicare, does not pay for all services after a patient has been discharged from the hospital. However, if something is determined by the doctor to be “medically necessary” you may be able to get coverage for certain skilled care or equipment. You will need to check directly with the hospital, your insurer, or Medicare to find out what services will be covered and what services will require you to pay out of pocket. Keep careful records of your conversations. The patient’s insurance company will be billed for services needed. If there is a co-payment required, arrangements will be made with our billing and finance personnel.
